2. Research Methodology

How We Conducted This Research

Between April and June 2026, DoubleSure conducted internal testing of healthcare-related search prompts across ChatGPT, Gemini, Perplexity, and Google AI Mode.

Testing scope:

  • Dozens of healthcare search prompts across multiple test rounds
  • Multiple specialties: physiotherapy, orthopaedics, dermatology, diagnostics, neurology, dentistry, pediatrics
  • Location-based searches across major Indian cities including Delhi, Noida, Ghaziabad, Mumbai, and Bangalore

For fifteen years, digital visibility for doctors meant one thing: ranking on Google's first page. The model was straightforward. Optimise your website. Build backlinks. Rank higher. Get clicked.

That model still works. But a new model is growing alongside it with meaningfully different mechanics.

Most clinics in India are strong at stage one — Discoverability. Their websites are indexed, their GBP exists, they appear on Practo. But they stall at stage two or three. Their websites have no schema. Their Practo listing uses a slightly different clinic name than their website. They have no editorial mentions. AI engines can find them but cannot confidently recognize them as a trusted entity.

The clinics that appear in AI search results have progressed through all five stages — not necessarily perfectly, but consistently enough for AI engines to recognize them as entities worth recommending.

5. What We Found When We Tested

Here is a simplified version of what a single query returned across all four engines simultaneously:

Results vary by date, location, and query phrasing. These reflect consistent patterns across multiple test runs, not a single isolated search.

Two patterns emerged across every category we tested.

Pattern 1: Third-party mentions outweighed first-party presence.

A clinic with a modest website but strong presence on Practo, a mention in a local healthcare publication, and active review responses consistently appeared more often than clinics with polished websites but no external footprint. In one observed case, a physiotherapy clinic with fewer than 80 Google reviews appeared in AI recommendations more consistently than a competitor clinic with over 300 reviews — because the first clinic had a Practo listing with 400+ patient recommendations, had been mentioned in two local health publications, and had consistent entity information across 30+ directories. The second clinic had more Google reviews but existed in only three places online.

Pattern 2: Structured data created a measurable difference.

Clinics with schema markup on their websites appeared more frequently and with richer descriptions in AI-generated answers than comparable clinics without it. Same clinical quality, same service offering — but a structural difference in how the web page communicated its content to automated systems. This pattern was particularly clear in Perplexity results, which showed a strong correlation between the presence of FAQPage and MedicalBusiness schema and inclusion in generated answers.

Neither finding is surprising in hindsight. But most healthcare practices haven't adjusted their digital strategy to account for them.

6. The Patient Discovery Gap Most Clinics Don't Know About

Two physiotherapy clinics operate in the same Noida sector. Similar services, similar pricing, comparable clinical quality.

Clinic A has a well-designed website, ranks on page two of Google for "physiotherapist Noida," and has 45 Google reviews averaging 4.6 stars. Clean site. Good reviews. No schema markup. No Practo listing. No mentions outside their own website.

Clinic B has an older website but appears on Practo with 400 patient recommendations, has been mentioned in a local healthcare publication's roundup, maintains consistent name-address-phone details across 30+ Indian directories, and has an active Google Business Profile with regular posts and review responses.

In traditional Google search, Clinic A's website quality and review count keep them competitive. In our AI search testing, Clinic B appeared consistently across all four engines while Clinic A barely registered.

Clinic A is doing nothing wrong by traditional metrics. The gap exists because AI engines recognize Clinic B as a known entity — something multiple independent sources confirm exists and is credible — while Clinic A is effectively invisible to AI systems despite being a real, functioning practice.

Depending on the query, AI assistants may recommend specific doctors, clinics, or healthcare organisations based on publicly available information. When they do recommend a specific practice, it is almost always one with a cross-platform presence that extends well beyond its own website.

7. Five Signals That Predict AI Search Visibility

Across our testing, five factors consistently predicted whether a clinic or doctor appeared in AI-generated answers.

Signal 1: Third-party editorial mentions

When an independent source — a healthcare publication, a patient community, a medical directory — mentions a clinic or doctor without commercial incentive, AI engines treat it as a high-value trust signal. This is distinct from paid placement or directory listings that function as advertisements.

The Indian healthcare ecosystem has several platforms that generate credible independent mentions: editorial features in publications like ETHealthworld, Express Healthcare, or regional healthcare magazines; patient community discussions on Quora and relevant Facebook groups; mentions in conference programmes and speaker profiles from bodies like the Indian Medical Association; and inclusion in NABH-affiliated directory listings.

One genuine editorial feature in a credible healthcare publication appears to carry more weight in AI search visibility than several months of blog posts on your own website. For those working on healthcare SEO, this external footprint is where the leverage is.

Signal 2: Structured data (schema markup)

Schema markup is the vocabulary that tells AI retrieval systems and search engines what a web page actually represents. Without it, a clinic's web page is just text. With the right schema, it becomes a machine-readable entity: a clinic named X, at location Y, offering services Z, with an aggregate rating of 4.8 based on 200 reviews.

Most clinic websites in India have none. A clinic that adds Physician schema (or MedicalBusiness schema), FAQPage schema, AggregateRating schema, and LocalBusiness schema with consistent NAP data makes itself substantially more readable to automated systems. What's changing is a layer being added on top — an AI-mediated layer that synthesizes information and makes recommendations rather than presenting a list of links.
